STUDENT-ATHLETE RANK ONE INFORMATION:
2025-2026 Pre-Participation Physicals: All athletes 7th- 12th grade will need an annual physical on file prior to any athletic participation. For PPE’s to be current for the 2024-2025 school year, they will need to be done AFTER May 1st, 2025 on the updated 2024 UIL PPE Form.

Rank One Student Online Medical Forms: All parents/guardians of athlete’s 7th-12th grade will need to complete the online medical forms prior to any athletic participation. Rank One forms for the 2025-2026 School Year will be ready to complete AFTER MAY 15TH, 2025. Any Rank One forms uploaded and/or completed prior to May 15th will not transfer over to the new school year and will need to be redone.
Uploading Physicals to Rank One: AFTER May 15th, please upload your completed physical form (both front and back pages) into Rank One while completing your other forms. By uploading an electronic copy, you will not need to bring a hard copy to your coach or the Athletic Trainers. Please double check that all sections have been completed and both Student and Parent/Guardian signatures are on the bottom of the front Medical History page.
